The Hunter New England and Central Coast Primary Health Network (HNECC PHN) is excited to announce the relaunch of the Health-e Together Digital Care Assessment (DCA)—an essential initiative designed to drive digital transformation across local healthcare services. Being implemented across General Practices in 2025, the DCA offers primary healthcare providers a unique opportunity to evaluate their digital capabilities and receive personalised support to advance digital health adoption.
Healthcare providers are invited to register their interest now, with the assessment set to begin on Tuesday 20 May.

Participants are placed into one of three maturity levels—Foundational, Intermediate, or Advanced—and receive a custom digital health action plan, including practical next steps and recommended resources.
